I’m sometimes asked which physicians earn the most. Here’s a look at how physician compensation is trending—and what it may tell us about the future of medicine.


This May 2026 article by Nelson, published in Medscape, highlighted findings from the Medscape Physician Compensation Report. Eight specialties averaged more than $500,000 annually: Orthopedics, Cardiology, Radiology, Gastroenterology, Plastic Surgery, Urology, ENT, and Anesthesiology. The report attributes these earnings to procedural volume, specialist scarcity, and technology-driven efficiency.


Radiology is also the specialty most frequently mentioned in discussions about AI. While artificial intelligence is already helping with image interpretation and workflow, it remains unclear whether it will meaningfully reduce radiologist compensation, increase productivity, or simply change how the work gets done.


The data suggest that highly specialized skills remain valuable, even in an era increasingly focused on automation.
